Child Profile Form

As with all clients it is important to tailor your scripts to match your client's issues and once completed, this form will allow you to refer to matters that are important in the child's life.

As long as the child is old enough to read, write and understand the questions (which are written in an easily understandable format), we also recommend that they be allowed to complete the information in their own words.
