Alcohol Awareness Form

Although not a formal diagnostic form, it can be used for information gathering to help devise the best treatment plan for your clients. The forms included enable clients to identify the situations, times as well as any emotional triggers where drinking is a problem and to keep a record of their progress. A questionnaire and a list of reasons to quit, together with a sheet of affirmations for the client to refer to, is also included.

By keeping a regular record of their alcohol consumption clients will become more self-aware and they and their therapist will be in a better position to help clients to eventually quit or cut down on their alcohol consumption. These documents can be downloaded in PDF or Word formats.
