Fear of earrings and pearls

Hi, I have a client who is so afraid of earrings that he can't even say the word. When he was trying to explain what it is that he is frightened of I asked if it was earrings and he immediately had to get a tissue and spit into it. There also seems to be a fear of pearls but much less so, he could talk about when his grandmother came to help him clean himself after going to the toilet when he was a small boy and her pearls would always fall into his face. He also has a fear of going to the toilet where anyone could know what he was doing, so public toilets, work toilets etc. he therefore has to make sure he has been to the toilet before he leaves his house in the morning. Any advice for me please?

It seems to be he has a shame issue, and associates pearls with the incident of his grandmother seeing him in a vunerable state.Have you regressed him, and talked through everything that happened? he may be leaving out something.Does he have any sexual problems aswell? just a thought.
