The resource section includes a selection of forms for Hypnotherapists to use in their practice.  With the exception of the Enuresis form, they are handed to the client at the start of the session to complete and give the therapist an insight into areas where intervention may be useful.  The forms are retained by the Hypnotherapist and should be destroyed when treatment is completed.

The Enuresis form is taken away by the client at the end of the session and filled in every day, so that he or she can see where progress is being made.  This form is returned on each subsequent visit and new ones can be given as required.

The Client Assessment Forms are essential for every hypnotherapist and a progress report should be made on the back of them, or on a separate sheet.  Relying on memory is not advisable as you may see several other clients in between sessions, making it difficult to remember what treatment has been offered.
